Development of a community health brokerage model to best meet the specific health and cultural needs of the Indigenous community of the Greater Bundaberg Region

Brief description:

To assist the Bundaberg Burnett Region Community Development Aboriginal Corporation (BBRCDAC), Indigenous Wellbeing Centre (IWC) to improve culturally appropriate access to mainstream primary health care services, and to purchase supplementary health services through appropriate brokerage arrangements

Outcome:

Phase 1- Model Development

  • Development of a demographic and health status profile of the Greater Bundaberg Region; mapping of available health services; identify service gaps, priority areas and key issues

  • Review of current services (as at December 2007) offered by the IWC, and of governance, management and administration; infrastructure; data management; operational components

  • Identification of enablers and barriers in the IWC’s current service provision

  • Interviews with key personnel to identify barriers & facilitators for Indigenous people accessing mainstream health services

  • Presentation of health broker model options, with recommendations for the development of a model most able to flexibly meet the growth of the organisation and the specific health and cultural needs of the local Indigenous community

  • Development of measurable milestones for implementing recommendations

Phase 2 - Interim Evaluation of Progress

  • Review of progress againt identified milestones

  • Identification and evaluation of issues affecting the organisation and consultation with Queensland Health to amend project plans towards ensuring continuing sustainability of the organisation

Phase 3 - Final Report

  • Review of progress against milestones and recommendations

  • Development of framework and procedures for emergency diabetes brokerage service

  • Discussion of outstanding issues
